It is our goal to minimize our patients’ stress relating to the financial responsibilities of fertility treatments. Our capable and caring financial and front office staff will be able to help you discern if you are responsible for any charges not covered by your insurance carrier. DOES MY INSURANCE COVER FERTILITY TREATMENTS?
While health insurance plans vary widely in their coverage of fertility-related services, most plans unfortunately cover a relatively small portion of patients’ treatment costs. Some plans even consider ‘diagnostic’ infertility services in a different reimbursement tier than ’treatment’ services of infertility.

WHAT CAN I DO TO FIND OUT ABOUT MY BENEFITS?
It is in each patient’s best interest to verify their coverage plan with their insurance provider prior to their first appointment at the Tree of Life Center. Please maintain a record of the person with whom you spoke, the date and details of your conversation. The staff at TLC will also verify your benefits at your first visit to provide you with a cross-check.

WHEN DO MAKE MY PAYMENTS?
All payments are due on the day professional services are rendered at the office. Payment for surgery is collected prior to the surgery date. Payments for ART procedures are made at the onset of each cycle. Refunds for cancelled services can be returned directly to the patient or kept at the Center for future treatment, as instructed by the patient.

CAN YOU EXPLAIN SOME INSURANCE STATEMENT COMMENTS ?
The amount paid by your insurance to you or directly to us for professional services received is dependent on your insurance carrier and the specific insurance plan you purchased. These benefits are specific to you and your insurance plan and bear no relationship to the actual value of the services provided at TLC, by TLC staff or by the Doctor. Notations such as exceeds usual and customary fees or more than the allowable charge are standard statements by your insurance carrier indicating they will not cover the entire bill. These notations are independent of the actual value of the services provided.

WILL I BE RESPONSIBLE FOR CHARGES NOT COVERED BY MY INSURANCE?
